Indian River County Fire Rescue provides ALS transport for Indian River County, Florida. The agency is tightly integrated with the two local hospitals and the two nearby trauma centers. It meets with the hospitals every month. They know the hospital staff including the CEOs on a first name basis.

Starting in 1994, the agency's paramedics activate the cath lab based on their interpretation of 12-lead ECGs. This practice is forced by the lacked of funding for transmitting ECGs wirelessly to the hospitals. Over time the false positive rate has decreased. The agency actively monitors STEMI activations and re-educates its paramedics as issues are discovered. A major part of the monitoring is accurate measurement of time intervals. The agency goes over the time stamps from the hospital to ensure accuracy.

It's estimated that 90% of stroke patients are transported by the agency.
