General Form

I think a good way to approach an international form is to combine all the possible values from primary, secondary and tertiary and let the user dictate the actual value since many of them repeat.

Mexican Form

What entry boxes would you like to see on a form to add information about a Mexican Ambulance Agency? For the U.S. form we have: City, County, State, Service Provider Type, Population, Service Square Mileage,

Would be great to provide both the Spanish and English word for both. Also, in the US the States and Counties control certifications, protocols, etc? How does this compare to Mexico? This would be helpful so we can have pages that describe regional system configurations. Thanks.

Every single agency in Mexico has a different Form, Since there is no actual authority to regulate the Pt. Data, all they do is provide a form that relates the general information about name, address, age, sex and weight, and then the form has boxes to fill in for the type of service it was provided, treatment and a box where you can input your narrative for the receiving physician at the Hospital. As soon as I can get a copy of a clean form for the FD I will posted in this discussion board. I believe that if its possible to start electronic documentation, that will be a much faster way to record all interventions and it will diminish the down time at the end of the call for this purpose.

Yes, I think the US is the same as far as patient data reports. I should have worded the question a little better. Here's a better way to ask it: Regionally, how is EMS set-up? By county? By city? If we wanted to make a page that could tell information about each Mexican EMS department/agency, what should it include? I looked up political divisions of Mexico, but I worry that, as a US citizen, I will overlook important info about Mexican EMS. So starting with the basics, to describe an EMS agency we would need: State, municipalities/municipios, etc. You can take a look at the U.S. form here Hocking County EMS. Thanks for the input so far!

besides the prehospital agencies, I would like to see how the hospitals are setup in Mexico. Do they have specific trauma centers? STEMI centers? Stroke centers? like we do in the US? I have always been curious. Being able to read it in Spanish and English would be awesome. I am trying to get some input from some of my connections. Will keep you updated!

United States Form

Any changes you would like to see on the U.S. Form?

Canadian Form

What entry boxes would you like to see on a form to add information about a Canadian Ambulance Agency? You can see an example of a US Agency here Hocking County EMS

Would be great to have the French and English word for both. Also, in the US the States and Counties control certifications, protocols, etc? How does this compare to Canada? This would be helpful so we can have pages that describe regional system configurations. Thanks
